Left lower lobe segmental bronchi

The superior segmental bronchus passes backwards from the top of the lobe; then anteromedial, lateral and posterior basal bronchi supply the base. The heart occupies the space that the right medial basal segment fills, which is why the left medial and anterior basal bronchi share a trunk.

Aspiration while lying on the left side
Superior or posterior basal segment of the left lower lobe; anaerobic abscess
Post-infective bronchiectasis or cystic fibrosis
Lower lobes and lingula are worst affected because gravity impairs drainage of the basal segmental bronchi; copious purulent sputum and coarse crackles
