Anatomical variant present in 0.1 to 2 per cent of people in which a right upper lobe bronchus, usually for the apical segment, arises directly from the right wall of the trachea up to 2 cm above the carina.

When it goes wrong

Endotracheal tube cuff sitting over the variant orifice

Recurrent right upper lobe collapse or pneumonia in a ventilated patient, or unexplained hypoxaemia after intubation
