Fibrous cord in the free edge of the falciform ligament from the umbilicus to the porta hepatis: the obliterated intra-abdominal umbilical vein, continuous with the ligamentum venosum.

Lies in the umbilical fissure separating the left lateral segments (II-III) from segment IV

Paraumbilical veins run alongside it, linking the left portal vein to the veins around the umbilicus

When it goes wrong

Portal hypertension in cirrhosis

Paraumbilical veins along the ligamentum teres recanalise, producing caput medusae around the umbilicus and sometimes a venous hum (Cruveilhier-Baumgarten)

Neonatal resuscitation or exchange transfusion in the first week

The vein has not yet fibrosed, so an umbilical venous catheter can still be passed through it
