Instructions

You are an intern in the emergency department. Your next patient is Hamish Dalgleish, 6, who has twice spat out fresh blood and clots tonight, six days after his tonsils and adenoids were removed here for loud snoring.

  1. Assess and manage Hamish using an A to E approach

  2. Outline to the examiner:

    • Next steps in management, including relevant investigations
  • Hamish is a manikin and his mother is with him; the examiner acts as the nurse and carries out your instructions
  • You do not need to summarise your findings or take a full history
  • The examiner will ask you questions at the end

Triage at 7.40 pm

HR 142, BP 94/58 mmHg, RR 26, SpO2 97% on air, T 37.9; weight 22 kg on the day of his operation
