Instructions

You are an intern on the postnatal ward at Geraldton Regional Hospital. It is 5 am, and the midwife asks you to see Theo Kyriakou, a term baby boy born 18 hours ago, who has started grunting, looks blotchy and has become floppy.

  1. Assess and manage the baby using an A to E approach

  2. Outline to the examiner:

    • Your findings
    • Your most likely diagnosis and differentials
    • Next steps in management, including relevant investigations
  • The baby is a manikin; his mother is at the bedside
  • The examiner acts as the midwife and carries out your instructions; describe procedures rather than doing them
  • You do not need to take a full history

Midwife's observations at 4.50 am

RR 70, HR 190, T 38.2 axillary; oxygen saturation and blood glucose not yet measured; birth weight 3.5 kg
