Lingular bronchus and segments

Segmental bronchi · Superior lingular Inferior lingular

The lingular bronchus leaves the left upper lobe bronchus and divides into superior and inferior lingular segmental bronchi. The lingula is the left homologue of the right middle lobe, lies against the left heart border, and has no fissure separating it from the rest of the upper lobe.

When it goes wrong

Lingular pneumonia or collapse

Loss of the left heart border on the PA film (silhouette sign); can be mistaken for cardiomegaly

Peribronchial nodes compressing the lingular bronchus

Middle lobe syndrome on the left: recurrent lingular atelectasis by the same mechanism as the right middle lobe
