Ilioinguinal nerve (L1)

Runs below and parallel to the iliohypogastric nerve, pierces internal oblique near the ASIS to enter the inguinal canal (it does NOT pass through the deep ring), lies on the spermatic cord or round ligament and leaves through the superficial inguinal ring.

Upper medial thigh, root of the penis and anterior scrotum, or mons pubis and labium majus
Reflex
Afferent limb of the cremasteric reflex

When it goes wrong

Caught in suture or mesh at inguinal hernia repair, or cut at a Pfannenstiel incision

Burning groin pain (post-herniorrhaphy neuralgia) with numbness of the upper medial thigh and anterior scrotum or labium majus; the cremasteric reflex may be lost
