A 54-year-old woman receiving adjuvant docetaxel and cyclophosphamide for breast cancer presents nine days after her second cycle with a temperature of 38.4 °C, rigors and lethargy. She has no cough, dysuria or diarrhoea, and her peripherally inserted central catheter site looks clean. Her pulse is 108/min and blood pressure 104/64 mmHg. Full blood count: haemoglobin 102 g/L, white cell count 0.9 × 10⁹/L, neutrophils 0.2 × 10⁹/L, platelets 96 × 10⁹/L. Chest radiograph is clear.
