A 61-year-old woman reports two weeks of brief episodes of the room spinning, lasting less than 30 seconds, that happen when she rolls over in bed to the right, lies back at the hairdresser, or looks up to hang out washing. Between episodes she is well. Her hearing is unchanged and she has no tinnitus, headache or ear discharge. Neurological examination including gait and cranial nerves is normal. When she is laid back quickly from sitting with her head turned to the right, after a delay of a few seconds her eyes beat in a twisting fashion for about 20 seconds while she feels intensely dizzy; on repeating the manoeuvre the response is weaker.
