Acute Caremiddle twist
A 4-year-old girl has had three days of watery diarrhoea and vomiting. She is irritable with dry mucous membranes and reduced urine output; her hands are warm, central capillary refill is under two seconds and her blood pressure is normal. She weighs 20.0 kg, down from 21.0 kg at a check last month. Nasogastric rehydration was started two hours ago and abandoned after she vomited each feed, so a cannula has been inserted.
