Acute Caremiddle twist
A 27-year-old man is brought in after his motorcycle struck a guardrail. A binder has been applied for an open-book pelvic fracture and he is haemodynamically stable after one litre of crystalloid. He has not passed urine in the six hours since the crash and his bladder is palpable halfway to the umbilicus. The nurse who cut off his clothing shows you the underpants he arrived in, which are stained at the fly, and his scrotum is swollen and dusky with the discolouration tracking back into the perineum.
