A 68-year-old woman is an involuntary inpatient at an authorised hospital in Perth with a severe depressive episode with psychotic features. She has not responded to two adequate antidepressant trials. She is eating and drinking, her biochemistry is normal and she is not in immediate danger. Her consultant proposes a course of electroconvulsive therapy starting on next week's list. She understands what is proposed and refuses it.
