An 80-year-old woman with moderate Alzheimer disease in residential care has been agitated and wandering at night for five days, and is drowsy during the day. Staff say she is 'not herself' and that this is new. She is afebrile, a bladder scan and bowel chart are normal, and a urine culture is negative. There has been no change to her room, her staff or her family's visiting. Her regular medicines are perindopril, atorvastatin and paracetamol; oxybutynin was started a week ago for urinary urgency.
