An 18-month-old boy is brought in after a generalised convulsion lasting about three minutes at home; he had a runny nose and was "hot" for a day beforehand. In the department 40 minutes later he is alert, smiling at his mother, and his temperature is 39.2 °C. Examination shows a red throat and bulging eardrums bilaterally; the fontanelle is closed, there is no rash, no neck stiffness and no focal neurological sign. His development has been normal. This is his first seizure, and his father had "fits with fevers" as a toddler.
