A 24-year-old man fell onto his outstretched right hand playing football two days ago. He has pain at the base of the thumb that is worse gripping. On examination there is tenderness in the hollow at the base of the thumb between the extensor tendons, and pain when the thumb is pushed along its long axis toward the wrist. Radiographs of the wrist, including dedicated views of the scaphoid, are reported as normal.
