A 4-year-old girl is found to have a heart murmur at a routine health check. She is thriving on the 50th centile, runs around the playground without becoming short of breath or blue, and has never had chest infections requiring hospital. The murmur is grade 2/6, soft, low-pitched and vibratory or "twanging", heard best at the lower left sternal edge in early systole, louder when she lies down and almost inaudible when she stands. The second heart sound splits normally with breathing. Femoral pulses are easily felt and there is no thrill.
