Acute Caremiddle twist
A 34-year-old woman arrives 20 minutes after eating a satay skewer at a food van. She has widespread weals, a hoarse voice and expiratory wheeze. Blood pressure 76/44 mmHg, pulse 128/min, oxygen saturation 94%. She is kept lying flat and is given adrenaline 500 micrograms intramuscularly into the mid-anterolateral thigh. A cannula is inserted and sodium chloride 0.9% is running. Five minutes after the injection she is still hoarse, the wheeze persists and her blood pressure is 80/48 mmHg.
