A boy is born at 39 weeks after spontaneous labour. The membranes ruptured 26 hours before birth and his mother had a temperature of 38.4 degrees during labour; no group B streptococcus swab had been taken in pregnancy. At four hours of age he grunts intermittently, his respiratory rate is 72/min and there is mild subcostal recession. He is alert, well perfused with a capillary refill time of two seconds, and his temperature is 36.6 degrees. Blood cultures have been taken and a lumbar puncture has excluded meningitis.
