Cardiovascularmiddle twist
A 34-year-old man who injects heroin presents with two weeks of fevers, sweats, a cough and pleuritic chest pain on both sides. His temperature is 39.2 °C, pulse 116/min. There are needle marks in both antecubital fossae and a healing abscess on the left forearm. Heart sounds are normal to your ear, with no audible murmur; the jugular venous pulse shows prominent waves coinciding with the carotid pulse, and the liver edge is tender and pulsatile. A chest radiograph shows several round opacities of 1–3 cm scattered through both lung fields, two of which contain air-fluid levels. Three sets of blood cultures grow *Staphylococcus aureus*.
