A 5-month-old ex-term boy is on day four of bronchiolitis. Over the past 24 hours he has taken about 40 per cent of his usual milk and had two wet nappies. Respiratory rate is 62 per minute with moderate subcostal and intercostal recession, and he pauses for breath while feeding. Oxygen saturation is 92 per cent in air, dipping briefly to 88 per cent during sleep and recovering without intervention. He is alert, well perfused and afebrile, with no apnoeas reported.
