Acute Caremiddle twist
A 41-year-old woman is bitten by her cat three hours ago. She has two puncture wounds over the dorsum of the right hand, one of them directly over the second metacarpophalangeal joint. There is no surrounding redness, she can move all the fingers fully and she is systemically well. As a teenager she developed lip swelling, wheeze and hypotension within minutes of taking amoxicillin. She is not pregnant. The wounds are irrigated and left open.
