An 89-year-old man with advanced dementia is in the emergency department with aspiration pneumonia. He cannot retain or weigh information about treatment. He made no advance health directive. His wife died three years ago; the brother he appointed as his enduring guardian died last year; his only daughter is on a cruise ship and cannot be contacted for a fortnight. His sister, aged 84 and of full capacity, is at the bedside with the neighbour who has washed and fed him daily for two years. Nasogastric feeding is proposed and is not urgent.
