A 4-month-old boy is brought in by his mother's partner with a swollen right thigh, which he says happened when the baby rolled off the couch this morning. On questioning, the injury may have occurred "yesterday, or the day before". The baby cannot yet roll. A radiograph shows a spiral fracture of the femoral shaft. On undressing him you find a yellow-green bruise over the left ribs and a fresh purple bruise on the right cheek. He is otherwise well nourished and afebrile.
