Acute Caremiddle twist
A 5-year-old girl weighing 20 kg is admitted with right lower lobe pneumonia. She is febrile at 38.9 degrees Celsius, saturating 93% on 2 litres per minute of oxygen by nasal prongs, and has refused all food and drink for 18 hours. She is warm and well perfused with a normal blood pressure and no postural symptoms. Serum sodium is 136 mmol/L, potassium 4.1 mmol/L and glucose 4.8 mmol/L.
