A 4-year-old boy has had two weeks of puffiness around his eyes that is worst on waking and improves during the day, and for the past few days his ankles have been swollen and his abdomen distended. He is otherwise well, with no rash, joint pain or recent sore throat. His blood pressure is 96/60 mmHg. Urinalysis shows protein 4+ and no blood. Results: serum albumin 16 g/L (35–50), cholesterol 9.2 mmol/L, creatinine 38 µmol/L, complement C3 normal, urine protein:creatinine ratio 620 mg/mmol (< 20).
