A 25-year-old male presents to the clinic with a gradual increase in pain and swelling on the right side of his scrotum over the last 3 days. On examination, there is tenderness on palpation of the posterior aspect of the right scrotum, which improves with testicular elevation. The cremasteric reflex is intact. The patient recently had multiple sexual partners and reports inconsistent condom use. He reports dysuria and urethral discharge. A urinalysis shows pyuria and bacteriuria.
