A 66-year-old man has had ninety minutes of central chest pain with sweating. His electrocardiogram shows 3 mm ST elevation in leads II, III and aVF with ST depression in I and aVL, and 1.5 mm ST elevation in lead V4R. His blood pressure is 88/56 mmHg and his heart rate is 56/min. The jugular venous pressure is elevated to the angle of the jaw and the lungs are clear on auscultation.
