A 78-year-old man with moderate Alzheimer disease is brought in by his wife, who is his only carer and is exhausted. For six weeks he has been getting up at 2 am, dressing, and calling out for his mother. He is not aggressive and has never left the house. Pain, constipation, urinary retention and infection have been excluded and his medicines are unchanged.
