An 80-year-old woman is admitted for assessment of cognitive decline. Her daughter says her memory 'fell off a cliff' in the five months since her husband died. She complains bitterly about her memory, answers 'I don't know' to most questions without attempting them, and is distressed by her failures. She scores 21 out of 30 on the Mini-Mental State Examination, but her attention and orientation are intact and there is no fluctuation. She wakes at 4 am, has lost 6 kg and no longer sees her friends. Computed tomography of the brain shows mild generalised atrophy only.
