A 46-year-old man taking venlafaxine 225 mg daily for depression is admitted after a fall at work. The orthopaedic team started slow-release tramadol two days ago. He is now agitated and drenched in sweat, with a temperature of 38.2 °C and a heart rate of 118 beats per minute. Both pupils are dilated. Dorsiflexing either ankle produces repetitive beating that lasts several seconds; his limbs are otherwise loose and his bowel sounds are normal. Creatine kinase is normal. Both drugs have been ceased. Which one of the following drugs should now be given?
