A 44-year-old man with treatment-resistant schizophrenia has taken clozapine 400 mg at night for six years and smokes 30 cigarettes a day. He is admitted to a smoke-free surgical ward for an inguinal hernia repair and accepts nicotine replacement patches. He will be an inpatient for four days and will not smoke during that time. His neutrophil count, temperature and electrocardiogram are normal and his bowels are opening daily. The surgical registrar asks whether the clozapine can simply be continued unchanged.
