You are the intern on an adult inpatient psychiatric unit. At the morning meeting the occupational therapist reports that a woman due for discharge tomorrow could not manage her own medicines when this was tested on the ward yesterday. The consultant replies that the bed is needed and moves to the next patient. Two days ago you watched the same woman tip her tablets into her hand and drop most of them on the floor, and you have not told anyone. The registrar says privately that the consultant does not like being contradicted. Which one of the following should you do?
