A 38-year-old man with bipolar I disorder has been well on lithium for two years. At review he says he has decided to stop it because of tremor and a 9 kg weight gain. He describes his two previous manic episodes, the job he lost in the second, and the high chance of relapse without treatment, and he still wants to stop.
