A 44-year-old man presents three hours after a sudden severe occipital headache that reached its worst within a minute while he was straining at stool. He vomited once. He is alert and orientated, has no neck stiffness and no focal neurological signs, and his blood pressure is 138/84 mmHg. A non-contrast computed tomography (CT) of the brain was performed two hours after the headache began, on a modern multi-detector scanner, and has been reported as normal by the consultant radiologist.
