A 46-year-old woman is admitted with two days of vomiting and lightheadedness. Her regular tablets are alendronate, esomeprazole and prednisolone, all of which she stopped herself a week ago. She is drowsy but rousable. Her pulse is 118 per minute and her blood pressure 78/44 mmHg, unchanged after two litres of compound sodium lactate. She is afebrile, the neck veins are flat and the lungs are clear. Sodium is 126 mmol/L, potassium 5.8 mmol/L and blood glucose 3.1 mmol/L. Which one of the following should she be given now?
