A 62-year-old man collapses in a hospital foyer and is found to be in ventricular fibrillation. Compressions have been continuous, adrenaline 1 mg has been given twice, amiodarone 300 mg followed by a further 150 mg has been given, and a supraglottic airway is in place. Three shocks have been delivered singly at the biphasic defibrillator's maximum energy, with the self-adhesive pads in the anterolateral position. He remains in coarse ventricular fibrillation after the third shock and the end-tidal carbon dioxide is 22 mmHg. Which one of the following may now be considered?
