A 79-year-old man, two days after hemiarthroplasty for a fractured neck of femur, is climbing over the bed rail and pulling at his cannula; he was lucid at the evening round. Nursing staff have moved him to a single room, dimmed the lights and stayed with him. Temperature is 36.8 degrees C, oxygen saturation 96 percent on room air, blood pressure 138/74 mmHg, capillary glucose 6.4 mmol/L.
