An 84-year-old woman was admitted three days ago with cellulitis, which is settling on flucloxacillin. Since yesterday she has been muddled at night, insists there are cats on her bed, and dozes through the day. She is afebrile; electrolytes, calcium, glucose, creatinine and full blood count are normal and urine dipstick is negative. She takes metoprolol, prednisolone 5 mg daily unchanged since 2019, and temazepam 10 mg nightly for twenty years, and has declined the tramadol charted for her leg. Her general practitioner started oxybutynin six days ago for urinary frequency. Which one of the following is the most likely cause of her confusion?
