An 88-year-old man on a medical ward has a delirium precipitated by pneumonia and is receiving appropriate antibiotics. Overnight he is terrified by figures he sees standing at the end of the bed, has pulled out his intravenous cannula and is trying to climb over the bed rail. The ward has been quietened, his daughter has come in to sit with him, his bladder is not distended and he is not in pain, but he remains frightened and unsafe.
