A 29-year-old man who has injected heroin daily for six years has completed a seven-day inpatient withdrawal. He is medically well and has declined both methadone and buprenorphine maintenance after a thorough discussion of each. He will return this afternoon to the share house he came from, where two housemates continue to inject. He has no general practitioner and no fixed follow-up.
