A 54-year-old man is admitted eighteen hours after his last drink, anxious, sweating and coarsely tremulous, with a pulse of 108/min and a blood pressure of 150/92 mmHg. He is orientated and has no hallucinations. He has known alcohol-related cirrhosis: he is jaundiced with spider naevi and moderate ascites, bilirubin 84 micromol/L (less than 20), INR 1.9 and albumin 26 g/L (35 to 50).
