A 63-year-old woman has taken slow-release morphine for eighteen months for pain from vertebral metastases, prescribed and reviewed monthly by her oncologist. She now needs a higher dose for the same relief, and when a dose was missed on the ward she became sweaty and restless with abdominal cramps. She takes it exactly as prescribed, has never requested an early script, uses no other substances, and her pain and mobility are clearly better on treatment than off it.
