A 23-year-old apprentice has had persecutory beliefs and voices commenting on his actions continuously for three months. He has stopped attending work and rarely leaves his room. During the second month he had a fortnight of low mood and poor appetite that has since settled, but the voices and beliefs continued unchanged throughout it and persist today. He has never had elevated mood or reduced sleep. Urine toxicology is negative and he drinks no alcohol. Physical examination and computed tomography of the brain are normal. Which one of the following is the most likely diagnosis?
