A 62-year-old man is reviewed in the cardiology clinic eight weeks after an anterior myocardial infarction. He meets criteria for a moderate depressive episode and agrees to start a drug. His echocardiogram shows a left ventricular ejection fraction of 34 per cent. His electrocardiogram shows a corrected QT interval of 486 ms. His blood pressure is 162/96 mmHg despite three agents.
