An 8-year-old boy is brought by his parents because two or three nights a week, usually about an hour after falling asleep, he sits bolt upright and screams. He is sweating with large pupils and a racing pulse, pushes his mother away and does not appear to know who she is. After about five minutes he lies down and sleeps. In the morning he is cheerful and remembers nothing of it. The episodes cluster after late-evening football training. Development, daytime behaviour and examination are normal. Which one of the following is the most likely diagnosis?
