A 5-month-old boy is brought by his mother, who has been told at the child health clinic that he is under the line and is frightened that something is being missed. He was born at term weighing 2.9 kg. His weight has followed a line just below the 3rd percentile at every visit since 6 weeks of age; his length is on the 10th and his head circumference on the 25th percentile. He is exclusively breastfed, feeds about three-hourly and has seven or eight wet nappies a day. He rolls both ways, smiles, laughs and reaches for toys. Examination is normal.
