A 31-year-old woman in her first pregnancy presents at 36 weeks with a frontal headache and "flashing lights" for six hours. Her blood pressure was 118/72 mmHg at booking; today it is 168/112 mmHg and, repeated after 15 minutes, 164/110 mmHg. Urine dipstick shows 3+ protein. Her knee reflexes are brisk and there are three beats of clonus at the ankle. There is tenderness under the right ribs. Results: platelets 88 × 10⁹/L (150–400), ALT 142 U/L (<40), creatinine 96 µmol/L, uric acid 0.48 mmol/L. The cardiotocograph is normal and the cervix is 1 cm dilated.
