A 68-year-old woman arrives at 10 pm vomiting, with a painful red right eye and rings around the streetlights. The right cornea is cloudy, the pupil is mid-dilated and does not react, and the globe feels hard. Intraocular pressure is 62 mmHg. She is laid flat and given acetazolamide 500 mg intravenously, an antiemetic, and topical timolol, brimonidine and prednisolone.
