A 79-year-old woman lives alone and has stopped driving after dark because oncoming headlights spread into a glare she cannot see through. She has given up the newspaper and has fallen twice in six months, once on the back step at dusk. Best corrected visual acuity is 6/9 in the right eye and 6/12 in the left. Both lenses show central opacities and the red reflex is dulled; the fundi and intraocular pressures are normal. She says she would have an operation if it would help.
